You need to fill up a self-reporting form and apply for exemption from institutional quarantine
As per the guidelines issued by Ministry of Health and Family Welfare, Government of India, all international passengers arriving in India are required to fill up a self-reporting form and undergo 14 days of quarantine (7 days of institutional quarantine followed by 7 days of home quarantine). However, passengers can seek exemption from institutional quarantine if they fall under one of the following categories:
Suffered a death in the family
Suffering from serious illness (Description to be provided)
Parents accompanied by children below 10 years
Covid-19 negative certificate (attach RT PCR test only)
To avail this exemption, passengers need to apply online through Air Suvidha at least 72 hours before boarding the flight.

The self-reporting form consists of four sections: personal details, contact details, travel details, and health declaration. You need to fill up all the fields with accurate and complete information. Some of the information you need to provide are:
Passport number, visa number, flight number, seat number, etc.

Phone number, email address, permanent address, etc.
Address of stay in India (hotel or residence)
Countries visited in the last 14 days and their duration of stay
Symptoms of COVID-19 (fever, cough, breathlessness, etc.)
History of contact with COVID-19 positive person or exposure to high-risk area
Any underlying medical condition or medication
You need to sign the form and submit it to the health and immigration counter
